Case Study: Care Coordination
PROFILE: Large privately-owned, multi-specialty physician group serving Georgia, Alabama and Tennessee.
OBJECTIVE: Improve the communication between care providers, payors and patients regarding the impact of care coordination, including improvements in ED utilization and other services.
CHALLENGE: The care coordinator encountered numerous obstacles to effective program management – including overall size of patient population, inconsistent data formats and related access, lack of automated processes – resulting in delays and inconsistent patient information.
SOLUTION: The PREMEDEX platform – using cloud-based technology – assigns unique patient identifiers to streamline updates from many sources. Automated data feeds are scheduled to import patient encounter updates in a timely manner. Smart queues automatically sort patient care requirements based on high-priority cases. Cases are segmented based on risk scores, which drive timing and form of communications.
RESULTS: Initial feedback indicates a significant improvement in process efficiency, largely attributed to automated data feeds and the implementation of real-time patient encounter dashboards. More timely, relevant patient interactions have resulted from coordinated updates managed through the PREMEDEX platform.
