Senior Independence is partnering with a leading Ohio accountable care organization to coordinate care and improve quality and patient satisfaction through a program called Home to Stay.
Home to Stay helps patients with complex care needs make successful transitions from hospital to home. After conducting a comprehensive review of their needs and available solutions, Senior Independence felt that the PREMEDEX solution provides the most complete set of capabilities that is also highly efficient for implementation and management of patient records and provider information.
